What is Enrolment?

To be eligible for enrolment you must be a New Zealand citizen or have a visa (work / student) valid for 2 years or more. 
In order for us to receive funding from the government for treating you, you need to have selected us (The Peoples Centre Trust, a primary health organisation) as your preferred care giver.  Once enrolled with us you can benefit from our many services including:

• $3 prescription fees - free for children under 6 years old.  (Prescription fees for casual patients are $15 per item!)

• Mental health and wellbeing services

• Diabetes Get Checked – annual comprehensive diabetes assessments with the nurse

• Care Plus – structured regular check-ups for people who require higher levels of healthcare

• Chronic care management – for people with chronic conditions requiring regular monitoring and management

You can only be enrolled with one PHO at any one time, and we ask that you transfer your medical records to us.

PATIENTS NEED TO BE AWARE THAT…..

We will not provide care for patients who are intoxicated with alcohol or drugs, who are abusive to staff and other patients, or whose behaviour is socially unacceptable.
